Acute Rehabilitation Program

 

Please note the Acute Rehabilitation Program and its 14 beds were moved to Freeport Hospital to make way for construction. The beds will move back and CMH will offer acute rehabilitation services in winter 2025, once the construction project is finished. The information on this page reflects the program as it will be when it opens up again.

Sometimes, an illness or injury may lead to a hospital stay for you to recover and rehabilitate. CMH offers intensive, short-term inpatient rehabilitation to help patients regain their strength and independence. 

The brand new, 14-bed rehabilitation unit offers general rehabilitation and stroke rehabilitation services as part of a regional program serving Waterloo and Wellington regions. This program is in partnership with Grand River Hospital – Freeport site in Kitchener and St. Joseph’s Health Centre in Guelph.

 

To better ensure recovery, patients are assigned to the first available bed at one of these locations. Once in an Inpatient Rehabilitation program, the health care team will talk to you about how long your stay might be. The goals are to promote healing, regain strength and mobility. Rehabilitation may continue in your home or another location as an outpatient once you leave the hospital.

 

General Rehabilitation - General rehabilitation is offered to people impacted by injury, disease and surgery. The goal is to help the person regain their best level of independence.

 

Stroke Rehabilitation – Stroke rehabilitation services are part of a Waterloo Wellington Regional Stroke Program. It is a specialised, high intensity program offered to people with stroke that is tailored to meet their achievable goals. The goal is to help the person regain their best level of independence.

Clothing: Bringing 2-3 changes of clothes are recommended. Choose loose fitting items, that are easy to put on.

  • Supportive Shoes (e.g. running shoes)

Personal Items: Please note this unit is “Scent Free.” The use of perfumes and scented items is NOT permitted.

Please bring any personal assistive devices that you require for your appointment. For example, any communication or mobility device you may need like a walker, hearing aid, etc.

 

Patient education for stroke

You are the most important member of your health care team. Your full participation is needed for you to gain the most from the program. You are encouraged to have your support person, and friends visit you in the evening, unless they are actively involved in your therapy.

 

The staff on the Inpatient Rehabilitation unit work together as a team. If needed, a meeting involving the support person, patient and the team may be arranged during your stay to discuss goals, needs, your progress, or discharge plans. Here are some of the team members and their roles.

 

  • Doctors (physicians): are responsible for your medical care.
  • Nurses: provide direct nursing care, support, and education. Your nurse assists you with the planning and coordination of your hospital stay and setting daily goals.
  • Occupational Therapist: assesses your activities of daily living (e.g., eating, bathing, getting dressed, etc.) and helps you to safely and independently do these tasks. Additionally, your therapist may recommend the use of assistive devices that will make these tasks easier to complete. The Occupational Therapist Assistant works closely with you and the Occupational Therapist to achieve your goals.
  • Pharmacist: monitors drug therapy and helps to identify actual and potential drug related problems. Your pharmacist can provide information to you and your support person, about the medicines prescribes to you.
  • Physiotherapist: Assesses your physical movement and helps you become more comfortable, safe and independent as you move. You may be recommended walking aids. Physiotherapy may also help manage pain. The Physiotherapist Assistant works closely with you and the physiotherapist to achieve your goals
  • Registered Dietitian: assesses nutritional issues and can provide education to you and your support person, about your individualized dietary needs.
  • Social Worker: assists in planning your discharge from the hospital, and may provide supportive counseling to you and your support person, as needed. Your social worker may also make referrals to community service providers as necessary.
  • Speech Language Pathologist: works with patients who have trouble swallowing and with communication (e.g. difficulty listening/reading, verbal expression/writing, voice/ fluency/clarity of speech). They also work to lessen cognition disorders.

The Rehabilitation Unit is located in Wing B, Level 3. The Wing B facility is easily accessible from both visitor parking lots. However, it might be best to park in Lot #2 as it holds many more cars.

 

  • If you park in Lot #2, you can access the hospital through the Main Entrance or the Emergency Entrance
  • If you park in Lot #4, you may access the hospital through Wing C.
