Referrals

Referrals need to come from your physician or nurse practitioner. Clinicians can make requests on-line or access referral forms in the DI and MRI Request for Consultation Forms section.

Walk-ins for x-rays with a requisition from your doctor are accepted, except for requests for Scoliosis, Leg Lengths and Skeletal Survey studies, which must be pre- booked.  All other consultation requests must be sent in by the referring physician or nurse practitioner. Once the requisition is received, you may be able to schedule the day and time of the session.

Individuals 50 years and over that meet the criteria, may self-schedule a mammography without a requisition from a referring clinician, as part of the OBSP program.

You can register for PocketHealth, which provides you with on-line access to your medical images. With PocketHealth, you will also be able to manage who sees the images. There is a nominal monthly fee that you can cancel at anytime.

Please note that accessing your medical records involves a different process. See the Health Record section for more information.
